Essential Records to Keep

  • Evidence of Coverage: You should have received Evidence of Coverage when your policy was issued. If you did not, request a copy by calling the customer service number on the back of your insurance card. It’s essential that you fully understand this information and save it in an easily accessible place.
  • Summary of Benefits: This summary provides you with a snapshot of your coverage that is easier to follow than your Evidence of Coverage.
  • Explanation of Benefits (EOB): This document details what the insurance company paid for a claim and your remaining financial obligation. You may receive an updated EOB after your insurance company pays the doctor, lab, etc. By looking at the claim number you can determine if it applies to the same visit. Make sure any co-pay you made is listed.

Important Risk Information for GAMMAGARD LIQUID

In clinical trials, two serious events (two episodes of aseptic meningitis in one patient) were deemed to be possibly related to the infusion of GAMMAGARD LIQUID. Various mild and moderate reactions, such as headache, fever, fatigue, chills, flushing, dizziness, urticaria, wheezing or chest tightness, nausea, vomiting, rigors, back pain, chest pain, muscle cramps, and changes in blood pressure may occur with infusions of GAMMAGARD LIQUID.
